Our dietitians
Our dietitians are all Accredited Practising Dietitians with Dietitians Australia. They hold Medicare provider numbers and welcome referrals under a GP Chronic Condition Management Plan (GPCCMP). We also have an Accredited Sports Dietitian (Sports Dietitians Australia) for performance, training and sports-nutrition referrals.

We also accept referrals for private patients, and provide evidence-based, individualised advice for patients with conditions including:
— Nutrition support for patients using GLP-1 medications (e.g. Ozempic, Wegovy, Mounjaro)

— RPAH Elimination Diet (the Failsafe Diet)

Medicare rebates
GP Chronic Condition Management Plan
Since 1 July 2025, GP Management Plans and Team Care Arrangements have been replaced by a single GP Chronic Condition Management Plan (GPCCMP). A GPCCMP with a referral to us gives your patient a Medicare rebate toward up to five allied health visits per calendar year (shared across all allied health providers).
Eating Disorder Plans
Patients with a diagnosed eating disorder may be eligible for an Eating Disorder Treatment and Management Plan (EDP), prepared by a GP, psychiatrist or paediatrician. An EDP provides Medicare rebates for up to 20 dietetic sessions within a 12-month period — separate from, and in addition to, the visits available under a GPCCMP. Patients diagnosed with anorexia nervosa are eligible; other eating disorders may qualify where the relevant clinical criteria are met. Our dietitians welcome EDP referrals and work alongside the patient’s medical and psychological treating team.
How to refer patients
No special referral form is required. Please send all referrals to our central referrals team.

Prepare the plan and referral
Prepare a GP Chronic Condition Management Plan and referral for your patient — or a referral letter for private patients.

We’ll allocate your patient to the most suitable dietitian and location so they’re seen as quickly as possible. If you’d like a specific dietitian, just note their name. Include relevant paperwork including recent pathology reults, current medications and the reason for referral.
